Why put it in writing

A written grievance creates a dated record that you gave your employer a chance to fix the problem. In constructive dismissal cases in particular, adjudicators usually expect that you used the grievance procedure first. Check your contract or staff handbook for the procedure and who to send it to.

I am asking for [the outcome you want, e.g. the behaviour to stop / payment of wages owed]. I would like a meeting to discuss this and a written response within [a reasonable time, e.g. 10 working days]. I would like to be accompanied at any meeting by [colleague / union representative]. If it is not resolved

Keep copies of everything. If the employer does not respond or the problem continues, you may have grounds for a WRC complaint, depending on what the issue is. Mind the 6-month deadline from the date of the contravention.
