- Your full name, address and contact details.
- Your employer's correct legal name and address. For a company this is usually the registered name ending in "Limited" or "Ltd". You can check it on the Companies Registration Office website. Naming the wrong entity is a common and avoidable problem.
- Your start and end dates, job title and pay.
- The date of the contravention (the dismissal, the missed payment, the incident). This sets your deadline.
- What outcome you are asking for.
Keep it short, factual and in date order. State what happened, when, who was involved and what you did about it. Leave out opinions and long background. A separate submission letter can carry the detail, so the form itself stays clear.
Most complaints must be made within 6 months of the contravention, with a limited extension to 12 months where there is reasonable cause. Filing close to the deadline is risky, so do not wait.
